Personal Data Processed

Niyazi Altıntoprak records and processes the following personal data belonging to patients: name, surname, Turkish ID number, passport number, temporary Turkish ID number, date of birth, marital status, gender, address, telephone number, e-mail address, bank account, IBAN number, credit card information, medical reports, diagnosis and examination data, biometric data, laboratory results, test results, physician analyses and comments, appointment information, prescription information, surveys, images, camera recordings (audio-video), and IP address-related personal data.

Purposes of Processing Personal Data

These personal data are processed for the purposes of conducting information security processes, training activities, activities in compliance with legislation, finance and accounting activities, monitoring and conducting legal affairs, communication activities, conducting / auditing business activities, ensuring business continuity, procurement processes for goods / services, retention and archiving activities, providing information to authorized persons, institutions and organizations, providing healthcare services, creating and monitoring appointment records, issuing prescriptions, conducting patient satisfaction activities, providing post-healthcare support and information, and conducting risk management processes.

Method and Legal Basis for Collecting Personal Data

The personal data in question are obtained verbally, in writing or electronically and processed wholly or partly by automated means, with all necessary technical and administrative measures taken for their protection. For invoices and payments, processing is based on the legal grounds under Article 5 of the Law that processing is expressly provided for by law, is necessary for the establishment and performance of a contract, or is necessary for fulfilment of a legal obligation. For social media applications processing data abroad for appointments, information activities, taking images and camera recordings, information activities and patient satisfaction processes, processing is based on the legal ground of obtaining explicit consent.

Processing and Transfer of Special Categories of Personal Data

Special categories of personal data relating to health and sexual life may be transferred without seeking explicit consent by persons under an obligation of secrecy or by authorized institutions and organizations for the purposes of protecting public health, preventive medicine, medical diagnosis, treatment and care services, and planning and managing healthcare services and their financing; otherwise, they are transferred where the data subject’s explicit consent has been obtained. In processing activities based on explicit consent, the processing activity is discontinued if explicit consent is withdrawn.

Transfer of Personal Data

Personal data are shared, limited to the realization of the purposes stated above, with natural persons or private-law legal entities, suppliers, authorized public institutions and organizations, organizations from which we receive contracted services or with which we cooperate, the website and social media.

Rights of the Data Subject

You may submit your requests within the scope of Article 11 of the Law regulating the “Rights of the Data Subject” by delivering the Application Form to Niyazi Altıntoprak in the manner detailed in the Niyazi Altıntoprak Personal Data Protection and Processing Policy available at www.niyazialtıntoprak.com.
